Change to community healthcare from 1 April

From 1 April 2025 community healthcare services in Bath and North East Somerset, Swindon and Wiltshire, will be provided by HCRG Care Group.

Community healthcare in Swindon is currently provided by Great Western Hospitals NHS Foundation Trust.

From 1 April 2025, the community healthcare you receive in your home or in another community facility in Swindon, will be provided by HCRG Care Group, instead of Great Western Hospitals NHS Foundation Trust.

You will continue to receive the same care and support from the same teams of healthcare professionals you usually see.

Your booked appointments won't change, but the location might and HCRG Care Group will contact you with any changes you need to know about nearer the time.

All care will continue to be provided at no charge to patients.

If you receive text message appointment reminders, they will come from HCRGCareGrp instead of NHSNoreply from 1 April 2025.

From 1 April 2025, HCRG Care Group will hold or have access to your personal health record, and where necessary, share information with partner organisations involved in your care.

This will ensure that professionals involved in your care have access to the information they need to provide safe and effective care.

HCRG Care Group is one of the UK’s largest providers of community-based care. They already provide adult community healthcare in Bath and North East Somerset and children’s community healthcare in Wiltshire.

Bath and North East Somerset, Swindon and Wiltshire Integrated Care Board has appointed HCRG Care Group to lead a new community-based care partnership with the NHS, local authorities and voluntary organisations.

Patients affected by this change will receive an email or letter in the post informing them of this change.
