New clients

For clients who are new to the service, a registered health professional can complete the Swindon Wheelchair and Special Seating Service Referral Form, to request a basic wheelchair or an assessment for more complex needs.

Existing clients

To request a re-referral for a client who is already known to the service and may already have had equipment supplied, a registered health professional can complete the Swindon Wheelchair and Special Seating Service Re-referral Form.

Re-referrals are also accepted from clients themselves, or from their representatives and completed forms can be emailed or posted to the Service (please see Contact).
