We appreciate that, if you have recently suffered the loss of a loved one, you may not be ready to talk about it.

But we would be grateful if you could spare the time to complete our online Evaluation of care of the dying form when you're ready.

We welcome comments and recognise the importance of monitoring the quality of the care given to patients in the final phase (the last two days) of their life, while at the Great Western Hospital, or in the community.

How to give us your feedback

Please complete either of these online forms, depending on where the death occurred:

If you would rather answer the questions over the phone, please contact our Chaplaincy.

Please leave a message and they will call you back.

If you're not ready to respond

If you would prefer to respond at a later date, even if it's some weeks or months following the bereavement, that's perfectly alright.

And, of course, we understand that you may not wish to respond at all.

About the survey

Please be assured that everything you tell us will be in confidence (you won't be required to provide any personal information).

Your responses and your comments will be collated along with those of other people and used to help to improve the care that seriously ill patients and their families receive at the Great Western Hospital.

Further information and support

If you need help to complete the survey or if you require a personal response, then please contact our Patient Advice and Liaison Service (PALS) Team.

Lots of help is available for people having to come to terms with a bereavement, whether they need practical advice or emotional support - please see Extra help: Bereavement Services.
