The Leadership Alliance for the Care of Dying People (LACDP) is a coalition of national organisations.
It was set up to lead on improving care for people at the end of their lives and their families.
The Alliance has published its report One Chance to Get it Right.
The report sets out the approach to caring for dying people that health and care organisations should adopt.
The Trust aims to provide compassionate care, tailored to the individual needs and wishes of dying patients and those closest to them.
The Trust welcomes the report and in response has developed a Personalised Care Plan and information booklet to support that care of dying patients and those important to them.
The plan is currently being piloted in the Great Western Hospital, with Trust wide roll out planned.
Care of dying patients is provided by Trust staff.
At the Great Western Hospital this care is supported by the in-reach Palliative Care Team provided by Prospect Hospice.
