Important information

The following information is for patients of:

  • Abbey Meads Medical Centre
  • Crossroads Surgery
  • Moredon Medical Centre
  • Penhill Surgery

If you are a patient of one of the above surgeries, your GP services are now being provided by Great Western Hospitals NHS Foundation Trust.

We are delighted to welcome you as a patient of the Trust and would like to introduce ourselves.

Bringing together GP, community and hospital services

As an NHS Foundation Trust, we are best known for our services at the Great Western Hospital.

You may be interested to know that we also provide community healthcare, in clinics and people's homes across Swindon.

These services are rated as 'Good' by the Care Quality Commission.

We have decided to bring together GP, community and hospital services, because this is a great opportunity to provide care which feels more joined up for patients and their families.

Improving the experience of patients

There is a lot of work ahead of us.

We are focused on resolving the significant challenges these practices have been facing, including difficulties with the appointment booking system and general GP recruitment.

It will take time to transform these services but we will be making changes which we are sure will improve the experience of the 30,000 people registered with these practices.

What next?

Patients should continue to book and attend appointments as normal.

We will soon be launching an updated website, where we will share our improvement plans.

You will also find information in the GP practices.

If you have any questions please speak to a member of staff.

Get involved

We are always keen to involve patients and their families in developing our services, so please look out for opportunities to share your views.
