What to expect


We have a Diabetes Specialist Inpatient Care Nurse, who receives referrals from the wards on a daily basis.

The Nurse will assess each patient's individual needs and will set up a plan of care in agreement with the patient.

The Nurse will then refer the patient on to other members of team in their care as required.

If you have been admitted to hospital with a diabetes-related foot problem, you will automatically be referred to the foot care co-ordinator.


You will meet a diabetes consultant or registrar, who will assess your individual care needs and may refer you on to a diabetes specialist nurse for education, support or treatment change.

You may also be referred to any member of the Diabetes Care Team depending on your specific care needs.
