The team supports patients' recovery at the Swindon Intermediate Care Centre (SwICC) and their rehabilitation at home.
It includes:
Together they work with stroke patients until they can take charge of their own lives again, and move on to enjoy life after a stroke.
The team works closely with other community services, such as reablement, other professionals and specialists within surrounding areas and in reach to other wards to provide stroke specific rehabilitation within patients own homes, or care homes
The Life After Stroke group sessions offer advice on anything from how to lower the risk of another stroke, to appropriate medication or information on local exercise and support groups.
This is a social group for male stroke survivors, which brings them together in a relaxed social environment where they can also work on woodwork activities.
The group meets on Wednesdays, 2.00pm-4.00pm at the community centre in Upham Road, Swindon.
