3Ts Drugs Formulary

The 3Ts Drug Formulary is used across the Great Western Hospitals NHS Foundation Trust, Swindon and North Wiltshire.

The formulary provides information on the medicines available in these areas and guidance on how they can be most appropriately used.

These pages give patients and the public access to all chapters of the formulary and all other relevant formulary documentation.

These pages are not designed to give patients advice on the appropriateness of their treatment and any questions you have regarding your treatment should be directed to the doctor managing your care.

Local NHS clinicians are directed to the fully interactive version of the 3Ts Formulary which can be found on the Trust Extranet.


The 3Ts Formulary is split into chapters to reflect the layout of the British National Formulary (BNF).

Shared Care Agreements

Shared Care Agreements give the hospital prescriber, the GP and the patient the information they need to make sure that the treatment is started and continued safely.
