Psychological support for patients with long-term conditions

Receiving a diagnosis can evoke a range of feelings for people, including:

  • Denial
  • Worry
  • Anger
  • Guilt
  • Sadness

Adjusting to life with a health condition is a process that can take time and effort, and making the necessary lifestyle changes to best manage the condition often requires a significant level of motivation and support from those around you.

Free support and advice is available (for patients registered with a GP practice in Swindon) through an NHS service: LIFT Psychology Swindon.

Referral

Patients can self-refer or arrange it through the LIFT Psychology Practitioner at their GP surgery.
