We appreciate that, if you have recently suffered the loss of a loved one, you may not be ready to talk about it.
But we would be grateful if you could spare the time to complete our online Evaluation of care of the dying form when you're ready.
We welcome comments and recognise the importance of monitoring the quality of the care given to patients in the final phase (the last two days) of their life, while at the Great Western Hospital, or in the community.
Please complete either of these online forms, depending on where the death occurred:
If you would rather answer the questions over the phone, please call:
The Pastoral Care Office Tel: 01793 604289
Please leave a message and they will call you back.
If you would prefer to respond at a later date, even if it's some weeks or months following the bereavement, that's perfectly alright.
And, of course, we understand that you may not wish to respond at all.
Please be assured that everything you tell us will be in confidence (you won't be required to provide any personal information).
Your responses and your comments will be collated along with those of other people and used to help to improve the care that seriously ill patients and their families receive at the Great Western Hospital.
If you need help to complete the survey or if you require a personal response, then please contact our Patient Advice and Liaison (PALS) Team.
Lots of help is available for people having to come to terms with a bereavement, whether they need practical advice or emotional support - please see Help and Support and Mortuary and Bereavement Services: useful links.
Please contact us at gwh.webmaster@nhs.net and we'll put it right.
