Dying Matters Week - 18-25 May 2015

Dying matters 200The Trust has joined forces with a range of health organisations to raise the profile of local End of Life Services during Dying Matters Week (18-22 May).

The aim of the drive is to engage clinicians and the wider public in the care of people in the last year of their life.

An Advance Care Planning document - to be held by the patient, allowing them to update and amend as they make decisions about what is important to them - will be launched during the week. 

People will also have the chance to complete the 'What matters to you' questionnaire (https://www.surveymonkey.com/s/H5S9KHQ) about a range of end of life issues to inform a local Dying Charter.

  • Healthwatch Swindon
  • Swindon Carers Centre

Advanced Care Plan Document Launch

  • At The Academy, Great Western Hospital, 12.00pm-2-00pm and
  • In the foyer at North Swindon Library in the Orbital Retail Park, Thamesdown Drive, 10.00am-3.00pm
