Community Nursing

The Community Nursing service provides care to patients at home or in residential homes, many of whom have long-term conditions or disabilities.  This includes assessment, advice and treatment

Community nurses will work with you and your family, often in partnership with other health care professionals.

The service includes support to people at the end of their life.

This service includes a triage team. same day response team and planned care.

If you think you or someone you are caring for needs a community nurse, please speak to your GP.

Community Matrons

The Community Matrons manage a caseload of patients with long term conditions and patients who are acutely ill but can be managed at home, preventing hospital admission.

Under the care of a Community Matron, individuals are provided wtih personalised care plans and intensive nursing in the patients own home, with referral to therapy and social care if required.

This service is provided for a defined period of time for each patient.
