Children's Outreach Nursing Service

The Children's Outreach Nursing Service at the Great Western Hospital is made up of a team of specially trained Nurses who care for children and their families in Swindon, both at home and in the wider community.

We can offer

  • Feeding management including assistance with nasogastric/gastrostomy feeding.
  • Care and monitoring of children requiring oxygen therapy.
  • Post operative nursing care.
  • Monitoring and administration of medication and IV drugs.
  • Palliative care and symptom control.
  • Care for children with complex health needs.
  • Specialist nursing care for children with cancer
  • Specialist nursing care for children with diabetes
  • Specialist nursing care for children with cystic fibrosis.

We

  • Work in partnership with families.
  • Provide a needs-led approach.
  • Co-ordinate care packages with other health care professionals and existing services.
  • Enable children to be cared for in the environment of their choice.

We also provide

  • Education and resources (including training for professionals as well as families and children).
  • Co-ordination of provision of equipment and supplies from Primary Health Care Teams.
  • Support for Health Visitors, school nurses, schools, playgroups and nurseries.
  • Liaison with inpatient and outpatient services.

Monday to Friday, 9.00am-5.00pm (excluding Bank Holidays)
