The registration process

The Medical Certificate of Cause of Death (MCCD) can be collected once the medical team have completed all documentation.

We will explain the process when you contact us, and will advise when the certificate will be ready for collection.

The MCCD should be taken to Swindon Register Office within five days in order to register the death, unless the Coroner is involved.

You may register the death in another district; this is called 'Death by Declaration', and it will take up to five days for the Death Certificate to be posted from Swindon Register Office.

Please liaise with your local Register Office if this service is required.

Extra Death Certificates

It's possible to buy one or more Death Certificates, which are often required for a range of administrative purposes such as dealing with the estate of the deceased.

Other forms

The Registrar will also give you two other forms:

Social Security certificate ('white form')

Please read the information on this form, fill it in and send it to your local Benefits Office so that the Department for Work and Pensions can deal with any benefits and/or state pensions.

Certificate for Burial or Cremation ('green form')

This is for your funeral director and authorises them to make arrangements on your behalf.
