"Today marks the start of an exciting new chapter for healthcare in Swindon, as our Trust has been named the new preferred provider of adult community healthcare services in the town.

"Swindon Clinical Commissioning Group selected us as the preferred provider following a long process in which a number of organisations submitted a plan of how they would provide the services over the coming years.

"Adult community healthcare services, which are currently provided by SEQOL, take place away from hospital and help people to stay well, while supporting patients who need ongoing long term care.

"Community nursing teams and out-of-hours GP support, as well as the care given to patients in Swindon Intermediate Care Centre (SWICC) and the Urgent Care Centre, all fall under the heading of adult community healthcare services.

This is great news for Swindon

"Being the provider of both acute and community healthcare, means that patients should experience a more joined up service, regardless of whether they are being treated in hospital, in their own home or elsewhere.

"We already have a proven track record of providing good care at community level through our previous work in Wiltshire and we are committed to working with our colleagues across Swindon to provide the same high standard.

"While our ambition is to join services together, help people to keep well and reduce system-wide pressures, we know these things won't happen overnight.

"Our main focus at this time is to make sure there is a smooth handover of this service and that patients continue to receive the care they need from the familiar faces they have come to know."

Thank you
